Previously the Council had led Cincinnati hospitals’ joint effort to improve quality and patient care under a collaborative initiative known as the Hospital Quality Improvement Project, which succeeded in vastly improving community core measure performance for high-cost, high utilization health conditions such as heart attacks.
As the project evolved, the Council and participating member hospitals have now made a commitment to drive measurable improvements in patient safety and quality through shared hospital accountability – particularly in these areas of work:
The group’s name has changed to Accountable Care Transformation Leadership (ACT) to emphasize this focus.
A specific cooperative goal for Council members is to reduce all-cause heart failure readmission rates for adults 18 and older by 10 percent by January 2012.
The Council’s improvement process begins with facilitating agreements among the hospitals on standard definitions and metrics, involves consistent collection and reporting of data, and includes best practice sharing.
ACT is committed to sharing and transparency. The ACT Committee, comprised of VPMAs, CMOs, medical directors, CEOs, quality directors and others, serves as the body with shared accountability for improving the quality of care in Council member hospitals.
