    Purpose

    To improve outcomes for high-cost, high-utilization health conditions through hospitals' increased compliance with best practices, and to empower patients to become active and educated participants in their health care.

    Description

    The Council leads participating hospitals' involvement in a joint effort with the Ohio Hospital Association that provides performance measures and tools for hospitals to use to improve the quality of care in their respective facilities.

    Hospital-specific performance information is available to the community at www.gchchospitalquality.org.

    Participating hospitals have succeeded in vastly improving community core measure performance for targeted health conditions over the last five years as they collaborate to learn from each other and make continual adjustments to further improve care.
