Purpose

To improve the transition in care for patients being transferred from a long-term care facility to a hospital and achieve time and resource savings by improving patient transfer communication.

Description

A Council-led initiative resulted in a consistent form that long-term care facilities can utilize to convey pertinent information when transferring a patient to the hospital.

The form, developed in collaboration by the Council’s Emergency Department Leadership Committee and the Affiliate Member Committee Advisory Board, provides a universal set of information that is helpful to the provider involved in the transition of care.
